Explanation registration form
You can submit a request to register as a new patient at the Lankhorst general practice via the online registration form. You can also pick up a registration form at the practice to fill in. It is very important that you call your previous doctor to request to transfer your file.
We accept new patients within a maximum of 10-15 minutes travel time from the practice, in Waalre and Aalst. We want to be able to reach you quickly in case of an emergency.
We offer you the possibility of an introductory meeting with your new doctor, please contact the assistant. She will work with you to find a suitable moment.
